ABSTRACT

Introducción: la depresión es un trastorno cada vez más prevalente alrededor del mundo. Los médicos generales son los profesionales de la salud más consultados por pacientes deprimidos. Más del 70% de los pacientes con depresión son vistos por médicos generales y no por especialistas en Psiquiatría. Según estudios realizados en Buenos Aires, más del 25% de los pacientes internados en Servicios de Clínica Médica en hospitales generales presenta depresión. Estos pacientes suelen ser atendidos y seguidos por médicos en formación, sean residentes o concurrentes de Clínica Médica. El objetivo del trabajo fue analizar el conocimiento sobre los inhibidores selectivos de la recaptura de serotonina (ISRS) que tienen los médicos residentes y concurrentes de Clínica Médica de 5 hospitales de la Ciudad Autónoma de Buenos Aires (CABA) y describir el tratamiento de un paciente depresivo por ellos. Material y métodos: se realizó un estudio descriptivo de corte transversal con un muestreo de tipo no probabilístico. Se utilizó como instrumento de medición un cuestionario semiestructurado organizado en dos secciones, una de datos demográficos que permiten caracterizar la muestra. La otra, de 15 ítems, explora los conocimientos sobre los ISRS y el tratamiento de la depresión. Dicho cuestionario fue revisado por 4 expertos. El instrumento es anónimo. Se aplicó a 59 médicos en formación en Clínica Médica, residentes y concurrentes, de 5 hospitales de la CABA, que participaron de forma voluntaria, durante el período agosto-septiembre de 2022. Resultados: la mayoría de los médicos en formación en Clínica Médica no tratan cuadros depresivos y, ante un paciente deprimido, solicitan la evaluación por un especialista en Salud Mental. Solo un 6,8% lo medica con un antidepresivo. Más del 75% de la muestra refiere recordar los conocimientos que tiene sobre de los ISRS de la cursada de Farmacología y un 13,6 de la cursada de Psiquiatría en la Facultad de Medicina. Conclusión: se observa un conocimiento deficitario sobre los ISRS en médicos residentes y concurrentes de Clínica Médica. Se considera necesario reforzar la formación sobre depresión y manejo de antidepresivos durante la residencia/concurrencia de Clínica Médica. (AU)


Introduction: depression is an increasingly common disorder around the world. General practitioners are the most frequently consulted health professionals by depressed patients. More than 70% of all depressed patients receive treatment by general practitioners and not by psychiatric specialists. According to studies conducted in Buenos Aires, more than 25% of all patients admitted to the Clinical Services in public hospitals present depression. These patients are usually under the care and follow-up of clinical trainee physicians, residents, or interns.This study aimed to analyze the knowledge about selective serotonin reuptake inhibitors (SSRIs) of clinical trainee residents and interns in five hospitals in the Ciudad Autónoma de Buenos Aires (CABA) and to describe their treatment of a depressive patient. Material and methods: we conducted a descriptive cross-sectional study with a non-probabilistic sampling. We used a semi-structured questionnaire arranged into two sections as a measuring tool. One, with demographic data to describe the sample. The other, with 15 items, explores respondents' knowledge of SSRIs and the treatment of depression. Four experts reviewed the questionnaire, which was anonymous. We applied it to 59 clinical medical trainees, residents, and interns from five CABA hospitals who volunteered to participate during August-September 2022. Results: most clinical trainees do not treat depressive conditions and, when confronted with a depressed patient, request an assessment by a Mental Health specialist. Only 6.8% medicate the patient with an antidepressant. More than 75% of the sample reported remembering their knowledge of SSRIs from the Pharmacology course and 13.6% from the Psychiatry course at the School of Medicine. Conclusion: there is a deficient knowledge about SSRIs in trainee residents and interns of Clínica Médica. We believe it is necessary to reinforce training on depression and management of antidepressants during residency/internship practice in Clínica Médica. (AU)

ABSTRACT

Objective:To survey the satisfaction and employment intention of general practice residents (GPRs) in Shanxi Province.Methods:A cross-sectional questionnaire survey was conducted in May 2021 among GPRs in Shanxi Province through the residency training information management platform. The questionnaire included general demographic information, clinical competency and independent practice ability, the status of the training base, their satisfaction with the training, career prospects and employment intention. The survey results were analyzed.Results:A total of 452 questionnaires were distributed, and 434 valid ones were recovered with an effective recovery rate of 96.02%. The age of respondents was (27.4±4.6) years, there were 155 GPRs (35.71%) enrolled in 2018, 131 GPRs (30.18%) in 2019, and 148 GPRs (34.10%) in 2020. A total of 286 GPRs (65.9%) thought that they were able to independently complete some procedures such as abdominal puncture, lumbar puncture, chest puncture, and bone puncture, and the proportion of GPRs who were able to complete the above procedures increased with the training time (49.3%(73/148), 70.2%(92/131), and 78.1%(121/155) in 2020, 2019, and 2018 batches, respectively, χ2=29.39, P<0.001); 372 respondents (85.7%) thought that they were able to independently receive patients with common diseases and make preliminary diagnosis and treatment, while there was no significant difference in the proportion of GPRs enrolled in 2020, 2019, and 2018 ( χ2=3.35, P=0.188); 425 GPRs (97.9%) expressed that the base arranged rotation according to the training syllabus; 390 GPRs (89.9%) thought that the proportion of knowledge and skill assessment in the final examination was appropriate. The survey showed that 70 GPRs (16.1%) thought that the number of cases and types of diseases in the teaching base fully met the requirements of the training program, 144 GPRs (33.2%) thought that mostly met the requirements, 197 GPRs (45.4%) thought that basically met the requirements, 22 GPRs (5.1%) thought that most of them did not meet the requirements, and 1 GPR (0.2%) thought that completely did not meet the requirements. Most of the respondents (368 (84.8%)) reported that they received guidance from clinical instructors for medical record writing; 174 (40.1%) and 172 (39.6%) GPRs were very satisfied or satisfied with general practice teachers and teaching base instructors; 207 GPRs (47.7%) were satisfied with accommodation (or subsidies), while 38 GPRs (8.3%) were very dissatisfied; 203 GPRs (46.8%) were satisfied with basic salary and benefit, while 35 GPRs (8.1%) were very dissatisfied. For general practice as the career, 88 GPRs (20.3%) liked it very much, 255 GPRs (58.8%) liked it, 67 GPRs (15.4%) did not like it very much, and 24 GPRs (5.5%) disliked it. For the prospects of general practice, 108 GPRs (24.9%) considered it very optimistic, 219 GPRs (50.5%) considered it relatively optimistic, 92 GPRs (21.2%) considered it not very optimistic, and 15 GPRs (3.5%) considered it very pessimistic. Regarding employment intentions, 174 GPRs (40.1%) intended to choose municipal hospitals, 170 GPRs (39.2%) to choose county level hospitals, only 9 GPRs (2.1%) to choose village clinics. Conclusions:The survey indicates that for GPRs the most dissatisfied aspects are the salary and benefit, as well as the living conditions during their training; after completing the contract, most GPRs hope to work in hospitals at county and above levels.

ABSTRACT

@#AIM: To investigate the application and effect of virtual-reality surgery exercise in minimally invasive cataract surgery training for ophthalmology residents.<p>METHODS:Twenty ophthalmology residents with equal seniority who had completed 3a standardized residency training in the Affiliated Eye Hospital of Nanjing Medical University from 2019 to 2021 were prospectively enrolled. After passing the theoretical examination, residents were randomly divided into virtual surgery exercise(Dry-lab)group(<i>n</i>=10)and real animal surgery exercise(Wet-lab)group(<i>n</i>=10). Dry-lab and Wet-lab group residents performed training using the Eye SI surgical simulator and pig eye respectively. At the end of the training, the overall training effects of the residents in both groups were rated using the Eye SI surgical simulator and the real pig eye operation, and the module training effects of the residents in both groups were rated using the virtual surgical simulator. Furthermore, a questionnaire survey was used to objectively evaluate the two training methods.<p>RESULTS:Residents in Dry-lab group had significantly higher total scores on both operation assessments,simulator assessment and real pig eye operation assessment than Wet-lab group(88.03±1.34 <i>vs</i> 80.35±2.87, 87.50±3.03 <i>vs</i> 77.60±5.62, 88.57±1.89 <i>vs</i> 83.10±3.22, all <i>P</i><0.01). The simulator module assessment results showed that the residents in Dry-lab group scored significantly better than Wet-lab group in terms of scores and completion time on each module(<i>P</i><0.01). The questionnaire results showed that Dry-lab group rated better than Wet-lab group in terms of the novelty of training, the proximity to the real surgical experience, the degree of help to the improvement of microsurgery skills, the confidence to perform real surgery, and the overall satisfaction of surgical training(<i>P</i><0.05). <p>CONCLUSION:Applying virtual-reality surgery exercise to cataract surgery skills training for ophthalmology residents can significantly improve the cataract skills, increase overall training satisfaction, and help residents enhance their confidence, psychological quality, decision-making, and processing ability during real surgery at the initial stage of practice. This provides a new standard and model for establishing a formal and standardized cataract surgery training system for ophthalmology residents.

Background@#Medical research is undoubtedly an essential tool in improving health care. Thus, every resident physician should participate in health research activities to keep his knowledge and training up-to-date. Although research is incorporated in residency training, several studies have shown that much is still lacking in terms of the residents’ knowledge, attitude, and practices in research.@*Objective@#To determine the knowledge, attitude, practices, and barriers in conducting research among resident physicians in Cagayan Valley Medical Center.@*Methods@#This study utilized a descriptive survey design. Purposive sampling with total enumeration was done. A total of 108 resident trainees were included in the study. Validated questionnaires were used to assess the residents’ knowledge, attitude, practices, and barriers in conducting research. @*Results@#The level of research knowledge of the resident trainees is below average, and their overall research practice is poor. On the contrary, the residents’ attitude towards research is positive. The residents’ level of research knowledge significantly differed across age (p=0.010) and year level (p=0.012). The trainees’ research attitude significantly differed in terms of current involvement in research (p=0.048). The top three barriers identified were lack of research training and skills, insufficient time allotted for research, and lack of statistical support.@*Conclusion@#There is a need to fill the gap between the residents’ positive attitude and their below average level of knowledge and poor practice in research. Addressing the barriers in conducting research is also needed to improve the overall standards of research among the residents.

ABSTRACT

Objective: To understand the anxiety and depression status of students before finishing the standardized resident training program (SRTP) and the influencing factors, so as to provide evidence-based basis for improving the mental health and management system for those residents. Methods: A questionnaire survey was conducted by combining the self-rating anxiety scale (SAS) and the self-rating depression scale (SDS). The participants were the graduating students of SRTP from two affliated hospitals of a military medical university, and they were surveyed and analyzed in Jul. 2018. Results: A total of 252 questionnaires were distributed and 252 valid questionnaires were returned, with a recovery rate of 100%. There were 123 (48.8%) males and 129 (51.2%) females, with an average age of (28.3 ± 2.1) years old. There were 87 (34.5%) undergraduate students, 138 (54.8%) master degree student, and 27 (10.7%) doctor degree students. Twenty-seven (10.7%) students were trained for one year, 96 (38.1%) for 2 years, and 129 (51.2%) for 3 years. There were 151 (59.9%) students from "985" universities or "211" universities (key universities in China), and 101 (40.1%) from other universities. Twenty (7.9%) participants were in military service. We found that 94.4% (238/252) of the graduating students were in an anxious state and 60.3% (152/252) in a depress state. The severity of anxiety was related to the length of training (χ2=14.214, P = 0.027). Depression was related to the type of university ("985" or "211" universities vs others, χ2 = 5.691, P = 0.017; military colleges vs regional colleges, χ2 = 5.069, P = 0.024). No relation was found between depression and anxiety state in the participants (r = 0.025, P = 0.696). Conclusion: Graduating students from SRTP are faced with multiple pressures such as medical job, employment and graduation, and the incidence rates of anxiety and depression are much higher than those of the general population and medical staff. Sufficient attention and intervention measures should be actively taken for prevention of depression and axiety.

Objective@#To observe the effectiveness of mini-clinical evaluation exercise (Mini-CEX) on the standardized training of residents in the department of nephrology.@*Methods@#Mini-CEX was used to evaluate the 80 residents who received clinical standardized training in the department of nephrology from March 2017 to March 2018 in our hospital. The assessment mainly contained seven aspects: medical history taking, physical examination, clinical diagnosis, therapeutic regimen, doctor-patient communication, humanistic care, and overall clinical ability. Meanwhile, a Mini-CEX questionnaire was conducted among 9 teachers and 80 residents, including their viewpiont and satisfaction degree on Mini-CEX. The SPSS 21.0 was used for statistical analysis and the independent sample t-test was used for comparison between the two groups.@*Results@#Through the Mini-CEX assessment, every aspect of clinical competence of residents at the end of this program was significantly higher than that at the early period, and the difference was statistically significant (P<0.05). The most obvious improvement in residents was their treatment plan, overall clinical ability and physical examination. About the assessment of therapeutic regimen, the average score at the end of the training was (7.525±1.062), which was better than before (5.175±1.035), and the difference was statistically significant (P=0.000). Besides, clinical diagnosis and humanistic care were significantly better than those at the beginning (P<0.05). About the assessment of clinical diagnosis, the average score after the training was (6.925±1.071), which was better than before (5.425±1.238), and the difference was statistically significant (P=0.000). According to results of Mini-CEX questionnaire, teachers thought that Mini-CEX was a better way of assessment (88.9%) and could promote self-learning (77.8%), while residents believed that Mini-CEX was close to clinical practice and the ward-or-practice-based assessment was relatively comprehensive, which could significantly improve clinical skills (85.0%) and comprehensive quality (87.5%), with a satisfaction degree of 92.5%.@*Conclusion@#Mini-CEX is applied in the standardized training of residents in the department of nephrology, which can effectively improve clinical comprehensive ability of residents and improve Self-learning of teachers.

ABSTRACT

PURPOSE: The aims of this study were to explore how oncologists and resident physicians practice end-of-life (EOL) discussions and to solicit their opinions on EOL discussions as a means to improve the quality of EOL care. MATERIALS AND METHODS: A survey questionnaire was developed to explore the experiences and opinions about EOL discussions among oncologists and residents. Descriptive statistics, the t test, and the chisquare test were performed for the analyses. RESULTS: A total of 147 oncologists and 229 residents participated in this study. The study respondents reported diverse definitions of “terminal state,” and mostrespondents tried to disclose the patient's condition to the patient and/or family members. Both groups were involved in EOL care discussions, with a rather low satisfaction level (57.82/100). The best timing to initiate discussionwas consideredwhen metastasis or disease recurrence occurred orwhen withdrawal of chemotherapy was anticipated. Furthermore, the study respondents suggested that patients and their family members should be included in the EOL discussion. Medical, legal, and ethical knowledge and communication difficulties along with practical issues were revealed as barriers and facilitators for EOL discussion. CONCLUSION: This study explored various perspectives of oncologists and resident physicians for EOL discussion. Since the Life-Sustaining-Treatment Decision-Making Act will be implemented shortly in Korea, now is the time for oncologists and residents to prepare themselves by acquiring legal knowledge and communication skills. To achieve this, education, training, and clinical tools for healthcare professionals are required.

For cultivating high-quality resident physicians with comprehensive theory, independent clinical thinking, great practical ability, and innovative scientific idea, it recourses to both optimizing training scheme and evaluation system with highly specialization and comprehensive quality. In this study, the authors proposed incorporate training patterns includingoptimiz-ing design, innovating training, and standardized assessment based on their recent experience on resident standardization training.

Resident standardization training is an important means of clinical physician training in our country. Critical care medicine has important status in the training process. It is the important link to ensure the quality of resident standardization training. Residents should grasp the identification and early detection of critical ill patients. Residents should also get the ability of general basic management for critical condition and the doctor-patient communication ability. In practice, we have explored the training mode of standardized training of resident doctors in critical care medicine by developing detailed training outline, a variety of teaching methods and emphasizing the cultivation of clinical work ability.

ABSTRACT

Standardized training of resident doctors is the golden stage for clinicians to establish good clinical thinking. Although respiratory disease is common, there are serious and critical diseases, which need to develop clinical thinking and improve the analytical ability. The resident physicians' instructor should help residents to establish the correct diagnostic thinking through the following means: clinical demonstration teaching show, guidance of reading the relevant literature and books, strengthening the basic skills of collecting patients' history and physical examination, and guidance of appropriate use of auxiliary examination and analysis of the results. For the treatment thinking, solid theoretical knowledge and life-long learning ideas and habits can improve the successful rate of initial treatment. Furthermore, being familiar with the efficacy evaluation system and analyzing the reasons for the success and failure of the treatment will help the residents to develop a good therapeutic thinking. The good clinical thinking established by the residents in the department of respiration will be conducive to their own follow-up specialist training and career development.

ABSTRACT

<p>We conducted a survey of Kampo medicine prescribing rates, and Kampo medicine awareness in first- and second-year resident physicians at three general hospitals in Hiroshima, Japan, to inform future Kampo medicine education strategies.<br>Overall, 85.7% of the resident physicians were interested in Kampo medicine, and 70.8% wanted to practice it. Furthermore, 57.8% of the resident physicians had prescribed Kampo medicine, and of these, about 30% were prescribing it a few times per month, with both the percentage of prescribing physicians and the frequency of prescription being higher than expected. In total, 90.8% of the resident physicians wanted to observe outpatient Kampo medicine clinics and learn about Kampo medicine in their postgraduate clinical training.<br>Overall, 97.4% of the resident physicians thought that seminars and lectures on Kampo medicine are needed, choosing responses that Western medicine is insufficient in some cases (27.0%) and that Kampo medicine is better than Western medicine in some cases (26.4%) among the choices for answers to the need for seminars and lectures. Meanwhile, the reasons for not having an interest in Kampo medicine were : having fewer opportunities to study it ; and feeling that it has a negative image (seeing it as outdated and unscientific). We think that appropriate Kampo medicine education for these physicians would reduce its negative image. The results of this survey suggest that resident physicians need a substantial education in Kampo medicine.</p>

ABSTRACT

Picture archiving and communication system (PACS) was used to perform archiving and retrospective cases report presentation in resident radiologist standardized training, and the current conven-tional article reviewing case report presentation form was compared. The results showed the scores of quiz and questionnaire of using archiving and retrospective cases report based on PACS were higher than using the current conventional form (P<0.05). This type of case report presentation can improve overall capability of resident radiologist standardized training including imaging diagnose,research and educational skill.

Objetivo: Caracterizar la violencia laboral presente en las diferentes sedes del Programa de Especialización en Obstetricia y Ginecología de Caracas según la percepción del residente. Métodos: Se realizó estudio prospectivo, descriptivo, de corte transversal, que incluyó 120 residentes del Programa de Especialización de Obstetricia y Ginecología durante el período comprendido entre julio 2012 y julio 2013. Previo consentimiento informado se aplicó una encuesta anónima a residentes seleccionados mediante una tabla de números aleatorios. Resultados: La frecuencia de violencia laboral fue de 97,5 % (117), prevaleciendo entre las víctimas el tipo de violencia institucional (96,6 %) seguida por la violencia psicológica (92,3 %) (P= 0,03). No se demostró relación entre la violencia laboral y el sexo (P= 0,713) o año de residencia de posgrado (P= 0,571) de las víctimas ni sede hospitalaria encuestada (P= 0,146). Los perpetradores de los actos de violencia identificados fueron el residente de año superior, médicos especialistas, familiares de pacientes y pacientes, entre otros. Conclusiones: Existe un alto índice de violencia laboral en el Programa de Especialización de Obstetricia y Ginecología, predominantemente institucional y psicológico que puede conducir a consecuencias adversas entre los residentes de posgrado.


Objective: To characterize workplace violence present in the several branches of the Caracas Ginecology and Obstetrics Specialization Program as perceived by the resident. Methods: Prospective, descriptive, cross-sectional study that included 120 residents of the Ginecology and Obstetrics Specialization Program for the period between July 2012 and July 2013. Informed consent an anonymous survey of residents selected using a random number table was used. Results: The frequency of workplace violence was 97.5 % (117), prevalent among victims of institutional violence type (96.6 %) followed by psychological violence (92.3 %) (P = 0,03). No relationship between workplace violence and sex (P = 0,713), year of graduate residence (P = 0,571) of the victims or respondent hospital (P= 0,146) was demonstrated. The perpetrators of violence were upper year resident, attendant, patient’s family and patient, among others. Conclusions: There is a high rate of workplace violence in the specialization of Obstetrics and Gynecology, predominantly institutional and psychological that may lead to adverse consequences among residents graduate.

ABSTRACT

Objective To sum up the status and existing problems of standardized training of resident physician in China,and to promote residency standardized training.Methods With essayas retrieving items,resident + standardization training as the search term,all relevant papers published in CNKI from January 2004 to January 2014 with a title containing residency standardized training were reviewed.By means of literature analysis,the literature in 8 aspects was classified including the year of publication,status of journal,training base selection conditions,training objects and training time,content,specialist fields,management,assessment methods and shortages.Excel was used to make descriptive analysis of the results.Results After elimination,we took 152 papers as experimental group.In detail,12 literatures about training base selection conditions demonstrated that the level and type of hospital was the main criteria for training base;28 literatures about training objects and training time indicated that trainees were oriented to the pattern of 5+3,the latitude of PhD and Masters could be decreased properly; 36 literatures about training content focused on clinical skills and expertise.Only 19 literatures was concerned with specialist fields,which needed enriching.12 articles about management implemented management of supervisor responsibility system.22 literatures about the assessment method recommend that priority should be given to clinical skills examination to explore the hospital problems of resident standardization training.Conclusion The standardized training of resident physician has been carried out in China,but there are still some problems which need to be further improved.

ABSTRACT

México tiene una gran historia y tradición en la formación de médicos residentes. Pero ¿qué encontramos detrás de este proceso? Relaciones de poder implícitas y explícitas, inadvertidas o ignoradas por conveniencia por parte de las instituciones universitarias y de salud, con el agravante de olvidar su compromiso en la formación de mujeres y hombres profesionales. Los residentes son reducidos a un indicador más de recursos humanos para la salud. El médico residente en formación académico-científica está sumergido en esta vorágine deshumanizada y termina por convertirse en un individuo para el cual el dominio del conocimiento es un instrumento de poder. En este proceso olvida que su actuar y su razón de ser radican en el principio de primun non nocere y de servir. El artículo describe la realidad actual de los residentes en México y propone algunas medidas para mejorar las condiciones en las cuales se desempeñan.


Mexico has a great history and tradition in relation to the training of resident physicians, but what we find behind this process?, Power relations implied and not implied, unnoticed or ignored for convenience by the academic and health institutions, with the aggravation of forgetting its commitment to the training of men and women "professionals" and limited to meet another indicator of "human resources for health." The resident physician in academic and scientific training is immersed in this dehumanized maelstrom and ends up becoming a character for the domain of knowledge as power, forgetting that his act and its rationale lies in the principle of "primum non nocere" to that we would add: nor your person, nor your fellowman, much less whom you have the moral, ethical and civic responsibility to convey some of your knowledge and your experience, that is, part of your essence”.

ABSTRACT

Objective To explore the effect of applying problem-based learning (PBL) combined with ' pre-resident physician' teaching model in clinical practice in the nephrology department.Methods Sixty students in clinical practice were divided into two groups:experimental group and control group.The students in experimental group were received PBL in combination with 'pre-resident physician' teaching model while those in control group were received traditional teaching model.The effect was evaluated according to the results of theoretic examination and investigation.The results were analyzed by x2 test and P < 0.05 signified statistically significant.Results The average score of theoretic examination in experimental group was 80.22 while that in control group was 76.53.The students in experimental group had significant higher score than those in control group (P < 0.05 ).The overall satisfaction degree was 93.00% in experimental group concerning the 8 indicators in PBL combined 'pre-resident physician' teaching model,while that was 73.75 % in control group concerning the 8 indicators in traditional teaching mode.Concerning the aspects of teaching flexibility,arousing learning interest,training literature capacity and improving ability of independent problem solving,the satisfaction degree was higher in experimental group than in control group (P < 0.05 ).Conclusion PBL in combination with ' pre-resident physician' teaching model has evident advantages in clinical practice in nephrology department.

ABSTRACT

The training of airway management plays a vital role in standardized residency training plan during the rotation in the department of anesthesiology.Problem based-learning and airway simulation practice should be applied step by step based on the different requirements of standardized residency training plan.The training should be started from mask ventilation to endotracheal intubation,meanwhile clinical thinking ability and teamwork ability should be cultivated for the residents so that they can master the technique of airway management skillfully whenever and wherever.
